One question Im always asked about when it comes to health care or senior care and social media is, What about HIPAA Online privacy and security are important issues, especially on the new social Web. In fact, its all about common sense. Below I provide a brief overview of the main issues, three examples of health caresenior living organizations effectively using social media without violating HIPAA, and some resources for developing your own social media and online communications policies. HIPAA Social Media. The Health Insurance Portability and Accountability Act HIPAA requires that a patients identity and personal health information be protected also called Protected Health Information, or PHI. Health care providers who violate HIPAA can face stiff penalties, including fines up to 2. As a result, many care related organizations shy away from deploying social media, blogs, and online communities due to fear of HIPAA violations. This is unfortunate as these organizations are missing out on the many opportunities and benefits that social technologies offer. Senior living companies, homecare agencies, skilled nursing facilities, hospitals and the like, should feel free to engage in social media as long as mechanisms to secure PHI are employed. Information posted to blogs, Twitter accounts, Facebook pages, or online communities by health care companies should be prepared for public consumption and each organization engaging in social media should have policies and guidelines related to social media and online communications in place prior to deploying these technologies. Here are three examples of health care and senior care organizations using social media, as well as some social media policies and online communications guidelines for your reference Innovis Health. Innovis Health is a non profit hospital and health care organization based out of Fargo North Dakota. Innovis has a blog shown below plus Twitter, Facebook show below, You. Tube, and Flikr accounts. On the blog, Innovis publishes hospital news and general health news. This content is then syndicated out to their Twitter and Facebook accounts. The blog also features a Twitter feed showing Innovis Twitter activity in real time. The Innovis You. Tube channel features hospital commercials and local news coverage. Their Flikr account displays photos of their various facilities, their staff, and marketing images from different marketing campaigns. This complement of social media tools allows Innovis to broadcast their message, engage a following, and drive traffic back to their website. Theres very little patient specific information on any of their social media properties, however, the blog does feature two stories about babies that were born at the hospital during a March 2. The assumption here is that the parents gave Innovis specific written permission to publish these stories a must if you plan to publish PHI or patient identity. Innovis Health Blog. Innovis Health Facebook Page. Childrens Hospital L. A. Childrens Hospital L. A. actually encourages patients and families to tell their stories on the hospital website. The way they work within HIPPA rules is by employing a 3 step process where the person submitting the story must Review a Use and Access Statement Review the hospital HIPAA policy. Fill out a secure online form where the story, images, and any links are uploaded to and reviewed by hospital staff for approval prior to being posted to the site for public viewing. Childrens Hosptial L. A. Share Your Story Page. Childrens hospital L. A. Use Access Statement. This is a shrewd business move on the hospitals part because it allows them to publish vetted positive stories from actual patients which clearly has marketing benefits without violating HIPAA. Terrace Communities. Terrace Communities is a group of seven assisted living residences located in Maine, Vermont, New Hampshire, and Florida. Terrace uses a member only, public facing branded online community to connect residents, staff, and family members from all seven facilities. Members have unique profile pages and can participate in discussions, post to the group blog, add to the calendar, and upload photos to the galleries. Below you can see the community homepage with a photo of a Terrace resident and her son prominently displayed. Terrace Communities. Notice that, like Childrens Hospital L. A., Terrace Communities encourages their members to interact and post content to the community website. Also, as you can see from the screenshot below, Terrace regularly posts photos of and stories about residents, family members, and staff to their community site. One difference here is that residents of Terrace communities are NOT considered or referred to as patients. Therefore, while privacy and information security are issues for Terrace, HIPAA is not a major concern. Terrace does have each resident and family sign a generic photo release form allowing them to publish photos on their website and in marketing materials. Terrace Communities Cont. Conclusion. Blogs, social media, and online communities can be powerful tools for organizations to broadcast news and information, as well as engage prospects and customers. The vast majority of what health care and senior care organizations publish using social technologies has nothing to do with PHI. As long as organizations take proper measures to ensure that Protected Health Information is kept private and secure theres nothing to fear. If you are going to publish patient photos or stories, get a written release. Of course, its always possible that a rogue employee who doesnt follow the rules can violate HIPAA, but that can happen offline even more easily than online and should not be a reason to avoid getting into social media. Additionally, proper social media management allows companies to monitor posted content and quickly remove anything inappropriate. Tenor roles can be less dramatically complex and meaty than baritone roles, but musically in my opinion they tend to get the soaring tunes that folks tend to whistle afterwards. If you are a baritone who wants to play primary protagonists all the time, your options may be limited in opera and more plentiful in musical theater Curly, Lancelot, Marius or operetta. In my early struggles with being a baritone, I was hoping that I could be the next Thomas Hampson and get some of the rarely performed baritone versions of popular tenor roles tenor roles transposed Werther, but that was and remains unlikely. Questions to Ask Yourself. None of these questions are fool proof. Some baritones have high passaggi and some tenors have a hard time with high Cs, but these can get you thinking more clearly about who you are Are you uncomfortable or in pain If you feel sore when you sing, whether its high or low, then you may want to try something else. If A2 on the bass clef feels bad to you, then you may be a higher voice. But if E4 feels bad to you even after regular practice, then you may have a low voice. Where is your passaggio Where is the most unstable area of your voiceThats likely to be your passaggio. Usually, it is a good guide to help you decide whether you are a high or low voiced person. Normally, we speak of having two passaggio breaks the first break primo passaggio and the second break secondo passaggio with a zone in between zona di passaggio. For me, the zona di passaggio feels similar to singing on a tight rope where one wrong move will cause me voice to flip in and out of falsetto in a fluttery kind of way. It is also difficult to hear my own voice properly there. As you ascend, you will reach a point where you have to tilt your jaw up if you continue to sing in the exact same manner as you began. Thats your primo passaggio. A fourth above is your secondo passaggio. This doesnt work in 1. What is your most comfortable tessitura The best guide to your range is your tessitura. Thats where youre happiest singing with good technique for an extended amount of time. Even if you can manage to sing all the notes in a given tessitura, it must sound pretty easy for you, or the audience will have a kind of cognitive dissonance with your performance. You might be able to honk out a killer high note, but if you cannot actually sing in the tessitura required by a piece, then you arent actually that voice type. For example, the aria Dies Bildnis ist bezaubernd schn from The Magic Flute is full of notes that a baritone can sing. The highest note is a Ab. The tessitura, however, sits too high for a baritone, and it will sound difficult for the baritone to sing. He will also most likely tire himself out before the end of the aria. For me Im happiest singing in the baritone range. When I sang Nemorino, I did it, but it felt so incredibly difficult and stressful, while singing Almaviva a baritone role in Le nozze di Figaro felt great. This takes experience and lots of trial and error, but if singing stops being fun due to an incorrect choice, then its best to make a different choice. Dont get attached because it might be wrong for you right now, but theres nothing wrong in trying something out once. Your gut may be telling you something. If you feel inadequate after listening to a singer or a sense that it is totally beyond you, then maybe that repertoire is not right for you. Are you faking it You may not feel any discomfort or pain when you sing, but you may be faking it. Faking Baritone Faking baritones will make choices that appear to make their voices lower and darker than they naturally would be. The voice might be quite dark, but something wont ring true about it. Examples of baritone fakery include a tongue shoved into your throat or an overly lengthened vocal tract by shoving your lips outward. You may be pulling your top lip down to darken your sound. You may be modifying your vowels too early to create an artificially low passaggio. You may sound incredibly loud to your own ears but small voiced to everyone else. Baritones are able to have a clear phonation lower than most tenors. Baritones arent basses, and they dont need to sound like basses, but at G3 and lower down to Bb. It doesnt matter how dark the timbre is if the singer doesnt have this fundamental clarity. Naturally, beginning singers may have a hard time phonating clearly in any part of the voice, so some flexibility is required there. Faking Tenor Faking tenors will make choices that make their voices appear higher and lighter than they naturally are. They might be able to sing very high notes, but something will feel false. Faking tenors may have a larynx that is pulled up into the backs of their throats. To give the appearance of a high passaggio, they may keep their voices spread instead of modifying their vowels appropriately. This sound may not sound like singing at all and may be highly unpleasant to listen to though even the prettiest voice can sound unpleasant if its loud and in a small room. True tenor high notes must be connected to the body. A faking tenor can use a well rounded hard rock wail to sing very high notes, but it wont have the same rooting that true tenors have, and they wont have a smooth transition to the rest of the voice. A true tenor should be able to briefly grunt most of their high notes with full abdominal support, while a baritone will hit an early uppermost limit.
